Driver Test Form Request

Effective 4/30/2021, HB1715 repeals the enrollment and reading proficiency requirement to obtain a Learner Permit and/or Driver License so the School Form is NO LONGER REQUIRED.  

Please visit the Oklahoma Department of Public Safety for more information on the documents needed for your appointment for a Learner Permit or Drivers License.  

Documentation for a REAL ID Driver's License:
Yukon High can provide a proof of enrollment letter for the residency proof for those students trying to obtain their REAL ID Driver's License.  Please request this letter from the Counseling Office Secretary within 2 business days prior to your REAL ID appointment.
